The Opportunity Scholarship Program allows students who live in the attendance area of a school designated by the State of Florida as a failing school to transfer to a higher-performing public school (i.e., a school receiving a "C" grade or better). An eligible school is a school that is designated by the Florida Department of Education as having received three (3) consecutive performance grades of “D” (that is, for 2014, 2015, and 2016) or a performance grade of “F” for 2016.
Procedures: If a school is designated as failing, the student's parent or guardian is notified by mail. If the parent chooses to participate in the Opportunity Scholarship Program, the parent must submit an application.The parent may choose one of the following options:
You may choose to have your child remain at the current or projected school. Please be aware that resources may be available at your current or projected school that may not be provided at the school of choice.
You may choose to transfer your child to another public school that has been graded “C” or higher. Transportation will be provided for elementary and middle school students. If you decide to apply for a transfer, you should follow the directions on the application form.
The parent must fill out an Opportunity Scholarship Program Student Transfer Application and either mail or bring it to the School Choice Office in the Duval County Public Schools’ administration building at 1701 Prudential Drive, 32207. The deadline is Friday, August 9. Applications received after August 9 will be considered “late” and processed only if time and space permits. No applications will be considered after August 16. DO NOT FAX THE APPLICATION. You will be notified by a member of the transportation department’s call center regarding the 2013-2014 school assignment and bus information for your child.
If a parent or guardian fails to receive a pre-printed Opportunity Scholarship Program Student Transfer Application in the mail and would like to apply, download an application from www.duvalchoice.com and return it along with two forms of proof of address to the School Choice Office.
You may choose to transfer the student to another public school that has been graded 'C' or higher that is NOT in Duval County. It is the parent's responsibility to contact the other school district to determine if that district will accept the student based on that district's adopted criteria. If accepted by the other school district, the parent must contact the School Choice Office at (904) 390-2144 to obtain a release from Duval County. Transportation to the other school district will be the parent's responsibility.
