• STUDENTS RETURNING TO TWIN LAKES AS A NEIGHBORHOOD SCHOOL:

    If your student is returning to Twin Lakes Academy Middle School from a Magnet, Charter, or Special Assignment school, then you must first contact the School Choice Office at (904) 390-2082 to cancel their enrollment at the other school before they can be registered at Twin Lakes Academy Middle School.  
     
     
    IN COUNTY REGISTRATION REQUIREMENTS:
    1. Two pieces of residency proof in the parent/legal guardian’s name (one piece of proof must come from Column A): 

    Column A

    Column B

    ·         Jacksonville Electric Authority (JEA) bill or TECO bill

    ·         Automobile Insurance

    ·         Current Bank Account Statement (may block out account number)

    ·         Mortgage agreement (or closing statement)

    ·         Credit Card statement

    ·         Payroll Statement

    ·         Lease agreement (from a company, or *private individual)

    ·         Bank account statement

    ·         Current non-cellular phone bill

    ·         Driver’s License or State ID

    ·         US postal service confirmation  of address change

    ·         Vehicle Registration

     *Private lease agreement must be notarized AND two additional documents from columns B and C are required.

    1. Updated Shot record (2 doses of Varicella are required for all 6th and 7th grade students and a TDAP is required for all 7th and 8th grade students)
    2. IEP , EP , or 504 plans for scheduling
    3. Current year enrollments require a withdrawal form from previous school and all current year grades for scheduling and placement.

    2.      Report card showing promotion:

        6th grade - year end 5th grade report card

        7th grade - year end 6th grade report card

        8th grade - year end 6th and 7th grade report cards

    4.      A full body physical exam - submitted within 30 days of FL school entry (a full-body physical exam performed within the past year will be accepted) *Student registrations coming from within the state of Florida are excluded from this requirement )

    5.      Birth Certificate – original – not a copy

    Social Security Card (optional)

    6.      State Testing Scores

    7.      IEP, EP , and 504 plans for scheduling purposes

    8.      Current year enrollments require a withdrawal form from previous school and current year grades for placement and scheduling.
