Eligibility & Forms
To be eligible to participate in middle school athletics, a student must:
- be in the 6th, 7th or 8th grade
- maintain a 2.0 GPA
- not be 15 years old Before September 1, 2014
- have a completed Athletic Packet on file in the Athletic Office

The Athletic Packet must include the following:
EL2 - Physical Form
EL3 - Consent-Release, Concussion, Heat & Cardiac Arrest Form
Insurance Verification
Middle School Sportsmanship Contract
Age Verification - Birth Certificate
Other Documents if Applicable:
EL12 - Non Member Private School
