In order for a VPK application to be accepted, the following documents must be in hand at the time of registering:

• VPK Eligibility and Enrollment Certificate
• CSE Registration Packet
• Birth Certificate
• Social Security Card
• Completed Physical Form
• Up To Date Immunization Record
• Two Proofs of Address
