CSE Registration Packet will contain the following items:

• Blue Authorization for Emergency Care card
• New and Returning Student Registration Application
• Elementary Student Information Enrollment Form
• Student Health Questionnaire
• Student Records Request

Proof of address can be shown the following ways:

• Rental Agreement or Notarized Letter (if living at a friend, relative, etc. home)
• Utility Bill
• Voter Registration Card
• Bank Statement
• Driver's License with current address
