Student Health Screenings
In accordance with the provisions of the School Health Services Act 381.0056 and the Duval County Public Schools Health Services Plan, vision, scoliosis, hearing, growth and development (Body Mass Index) are screened. Populations targeted for mandated screenings are specified in Chapter 64F-6.003, Florida Administrative Code (F.A.C.).
- Hearing screening shall be provided, at a minimum, to students in grades kindergarten (K), 1 and 6; and students entering Florida schools for the first time in grades K through 5; and optionally to students in grade 3.
- Vision screening shall be provided, at a minimum, to students in grades K, 1, 3, 6 and students entering Florida schools for the first time in grades K through 5.
- Growth and development screening shall be provided, at a minimum, to students in grades 1, 3 6, and optionally 9.
- Scoliosis screening shall be provided, at a minimum, to students in grade 6.

Please Note: Student Health Screening is population based and done on all students designated to receive these screening services, unless parents opt out. Parents may opt-out of any or all screenings by writing DO NOT SCREEN in the boxes provided on the Emergency Contact Information and Authorization for Release form which is completed annually.
