Dear Parents, Coaches, and Student-Athletes,
Our goal is to provide the most up-to-date information regarding participation in athletics at Jean Ribault. For further questions, you may contact the head coach of your respective sports team or the head athletic trainer at burrought@duvalschools.org.
Required Paperwork for Sports Participation:
All student-athletes must have an up to date sports physical, referred to as form EL2 on the FHSAA website. This form must be signed off by a licensed healthcare physician clearing the athlete for full participation in sports. Along with the EL2, all student-athletes must have a complete EL3 form signed by the athlete and parent/guardian, a completed Health Insurance Verification form (a copy of the health insurance card is not required), Sportsmanship Contract signed by the athlete and parent/guardian, a copy of the athlete's birth certificate, and a completed Consent for Treatment form signed by parent/guardian. This packet is provided by the athletic trainer, coach, or athletic director. The entire sports packet is active for one year and must be renewed every 365 days. All required paperwork MUST be completed and turned in to either the head coach or athletic trainer before they can participate in any sports-related activity on campus. This includes pre-season conditioning.
Underlying Health Conditions:
For athletes with underlying conditions (including but not limited to: asthma, high blood pressure, sickle cell anemia/sickle cell trait, heart conditions, etc) must be cleared by their physician with a plan of care for managment of their underlying conditions when participating in sports. For example, athletes who have asthma must be cleared by their physician and must have an inhaler with them during all practices, games, or events. Athletes should communicate with the head athletic trainer and head coaches regarding any underlying health conditions they may have. The athletic trainer will help coordinate a plan of care with the athlete, coach, and parent during their participation.
Athletic Training Facility: Our Athletic Training Facility (ATF) is where our student-athletes can receive treatment, rehabilitation, taping, and injury evaluation from our athletic trainer. The ATF is located in the Trojans Field House, Room 002. Hours of operations vary (see below). All student-athletes must be accompanied by a coaching staff member or athletic trainer while in the ATF. Student-athletes must sign-in before receiving services. A maximum of three student-athletes are permitted in the ATF at a time.

11:00am - 1:00pm: Office Hours
1:00pm-3:00pm: Taping, treatment, and rehabilitation
3:00pm-6:00pm: CLOSED for practice coverage
6:00pm-6:15pm: Ice, treatment
Game Days:
1:00pm-3:00pm: Taping, treatment, and rehabilitation
4:00pm-9:00pm: CLOSED for practice/game coverage
Athletic Training Facility Policies:
- Sign in for AT services
- No profanity
- Respect yourself and others
- Must wear shirt, shorts/pants, unless otherwise permitted for treatment
- No cleats on training tables
- No more than three student-athletes at a time
- Clean up after yourself
- No students or athletes allowed in ATF without accompanied by staff (coach, athletic trainer, athletic director)
Anyone who does not adhere to our ATF policies will be asked to leave the facility.
