Registration Information for State Assessments 2022-2023
Please read through the following information carefully:
Registration:
- Students must be registered appropriately with the Duval County Public Schools Home Education Office or be enrolled in a Duval County private school.
- If enrolled in Home Education, the grade on this application must match the grade in Focus, the student information system.
- Private school students and students on an Empowerment Scholarship should be registered for state testing by their private school.
- The registration form must be properly completed and submitted no later than 4:00 p.m.by the deadlines indicated below in red.
Scheduling/Testing:
- We provide two testing locations based on zip code: Lake Forest Elementary and the Schultz Center. Students who live in the north and west part of Duval County will test at Lake Forest Elementary. Students living in the south and east part of Duval County will test at the Schultz Center.
- At check-in, each student must provide a student ID that includes his/her full name and a recent photograph. Acceptable forms of ID are passports, military IDs, school IDs, state issued IDs, or a completed Assessment Services Student Identification form. The Assessment Services Student Identification form will be attached to the State Assessment Registration Confirmation email you will receive after registering.
- Students must adhere to the DCPS dress code while at the testing site. Face masks are strongly encouraged.
- Students who arrive more than 30 minutes late will not be permitted to test.
- Parents are required to pick up their student within 20 minutes of the student completing the assessment.
Scores:
- FSA and EOC scores are typically released six weeks after the testing window closes. FAST PM scores are typically available within the Parent Portal 24 hours after the student has tested.
- Scores are not provided over the phone or via email from the Testing Office.
- Private school students’ Individual Score Reports will be sent to the school.
- Home Education students’ Individual Score Reports will be available in Focus. Please contact the Home Education Office (904-390-2477) when scores are released if you need assistance.
Consent to Receive School Health Services Form:
- New for the 2022-2023 school year, all students testing on Duval County Public School campuses will need to have a completed Emergency Contact Information, Authorization for Release of Student from School and Consent to Receive School Health Services form on file in order to receive any care and/or treatment for illness, injury and/or emergency care.
- Forms will be attached to the State Assessment Registration Confirmation email you will receive after registering.
- Please bring the completed consent form with you to check-in. A DCPS employee will collect it and keep it on file. Only one form is needed for each school year and will cover all testing windows within the school year. Families with multiple children testing will need to complete a form for each child. Do not email any completed forms to the State Testing Office as it contains confidential information and will be deleted.

Fall Assessments
Fall assessments begin September 6, 2022.

- Geometry EOC
- Civics EOC (middle school students only)
- Biology EOC
- US History EOC (high school students only)
Winter Assessments
Winter assessments begin November 28, 2022.
The deadline to submit the registration form is Tuesday, November 1, 2022.
Assessments available include:
- Star Early Literacy (Grades: K & 1)*
- Star Reading (Grades: 1 & 2)*
- Star Math (Grades: K-2)*
- FAST PM Reading (Grades: 3-10)*
- FAST PM Math (Grades: 3-8)*
- B.E.S.T. Algebra 1 EOC (only students who complete the course in December 2022 are eligible to test in this window)^
- B.E.S.T. Geometry EOC (only students who complete the course in December 2022 are eligible to test in this window)^
- Civics EOC (middle school students only)^
- Biology EOC^
- US History EOC (high school students only)^
- Florida Civic Literacy Exam (US Government students only)^
*These assessments will test the beginning of January 2023.
^Assessments will test the end of November/beginning of December 2022.
Spring Retakes
Spring Retake assessments begin February 21, 2023.
The deadline to submit the registration form is Tuesday, January 24, 2023.
Assessments available include:
- FSA Grade 10 ELA Retake (11th and 12th grade only)
- Algebra 1 EOC Retake
Spring Assessments
Spring assessments begin May 1, 2023.
The deadline to submit the registration form is Tuesday, March 21, 2023.
Assessments available include:
- Star Early Literacy (Grades: VPK & K)
- Star Reading (Grades: 1 & 2)
- Star Math (Grades: K-2)
- FAST PM Reading (Grades: 3-10)
- FAST PM Math (Grades: 3-8)
- B.E.S.T. Algebra 1 EOC

The deadline to submit the registration form is Friday, June 16, 2023.
