    1. Private Schools Consultation - October 1, 2020 - Income Determination Survey for participation for the 2021-2022 school year - The Title I, Part A consultation pertaining to the Income Determination Forms was presented during this meeting.  TITLE I, PART A
    2. Friday, November 20, 2020 at 8:00 a.m.This virtual meeting will provide information and an update on the CARES Act funding.  Additionally, Private schools will receive program overviews and participation requirements for Title I, Part A; Title II, Part A; and Title IV, Part A.  This overview of the upcoming year includes an explanation of participation requirements, date certain requirements, funding, and deadlines.  Schools that are currently participating in the grant projects will be provided with program updates for successful school implementation and parent engagement.  If you are unable to attend the meeting, please allow a representative to log in on your behalf.

    Schools will NOT be able to participate in the 2021-2022 grant projects without completing all of the required forms.  Please pay close attention to the due dates and the requirements.  Feel free to ask questions about what is required as participation documentation is provided in advance.  We will follow up with question and answer sessions and provide technical support.  

    Title I, Part A

    Please note, multiple steps and documents are needed for participation in federally funded programs.  A complete participation packet is needed for eligibility.  Please follow all steps below for the desired program.  These steps were agreed on in consultation.
     

    1. Intent to Participate Form Due:  February 19, 2021

    3. Evidence that the school is a non-profit organization by February 19, 2021
    4. Title I, Part A - Digital Income Determination Form - Schools who would like to participate in Title I, Part A for the 2021-2022 school year should be in the process of collecting Income Determination Forms from all parents.  The due date for these forms from ALL parents is February 8, 2021 for participation in the Title I, Part A program.  The form can be collected either digitally or via paper. Digital Income Determination Form or Paper form - Income Determination Form
    5. Title I, Part A - Title I Service Delivery Plan - This form documents the mechanics of how services will be delivered at the school site for the 2021-2022 school year.  This will be completed prior to the start of the 2021-2022 school year in collaboration with the Local Education Agency.  
    6. Title I, Part A - Title I Private School Parent Consent Form - This form is due from all parents of students who are participating in Title I, Part A services by August 1, 2021.  For students who are new to the school, it is due by Labor Day.  It is essential for services to start on the FIRST DAY that school starts for the traditional public school on August   
    7. Title I, Part A - Title I Student Referral Form - This form is to be completed by teachers or administrators for all students who will receive Title I, Part A services at the beginning of the school year.  For students who are new to the school, their forms should be submitted by Labor Day.
    8. Title I Pooling Survey, extended due date to June 11,2021 - https://dcps.qualtrics.com/jfe/form/SV_9LvUfcawL2G9s34  Pooling is a process that assists with teh distribution of funds to participating schools.  
    9. Title I 3rd Party Contractor Selection extended to June 11, 2021 - https://dcps.qualtrics.com/jfe/form/SV_2nKAGJAAcBE1EeG Duval County has two companies currently scheduled to provide services.  Schools will use this link to make a selection.
