    ATTENTION CLASS OF 2021: Final transcripts will not be available until they are released by the District. This date is still TBD (to be determined). Please note that transcripts are typically not available until the 2nd or 3rd week of June. Thank you for your patience and understanding.



    At this time, because of the CDC recommendations regarding COVID-19 and social distancing, ALL transcript requests for current AC students and graduates from the years 2016 to 2019 must be sent to the Records Clerk, Ms. CJ Frohock via e-mail at Frohockc@duvalschools.org. Please include the following information in your e-mail request: student name, date of birth and/or student ID number.  Don’t forget to include the requested schools.  

    If you are requesting your transcripts to be sent to a public college or university in the state of Florida via the State of Florida FASTER system, you may download the request form, complete it and either e-mail it to the Records Clerk, Ms. CJ Frohock at Frohockc@duvalschools.org or fax it to the records department in the Guidance Office at 904-348-5313.  The best form of communication at this time is via e-mail.

    2015 AND PRIOR ALUMNI/FORMER STUDENTS, graduates and non-graduates must request transcripts from the Office of Student Records via the Scrib-Order Online Student Record System through the link found here: https://dcps.duvalschools.org/Page/15018.  Please call 904-390-2088 for more information.


    Records and Transcripts FAQ and Procedures
    How do I request transcripts?

    ALL transcript requests must be made through the Student Services Office in the Media Center, in person, on school days, during student lunches and before or after school by completing a Transcript Request Form available in the Media Center at Student Services.
    How long does it take to receive my transcript or record request?

    ALL Record requests, including transcripts, take 3 days to prepare and proper identification is required at pick up.  Please pick them up in the Main Guidance Office, Room 102.  There is a $3 transcript fee for each hard copy. State of Florida Public Colleges & Universities accepting transcripts through the Florida FASTER system will be sent FREE of charge.
    Are Transcripts mailed, faxed or emailed?

    No. Transcripts are not e-mailed, faxed or mailed (except final to accepted college of choice post-graduation in June). 
    Are Electronic Transcripts sent to public Florida Colleges?

    Yes, but you must complete a Transcript Request Form and submit it to Student Services, located in the Media Center.
    How will you receive my Florida Virtual School Grades?  

    You must print out and bring your Florida Virtual School grades in to your guidance counselor. These grades/records are NOT auto-generated into our system from FLVS and may delay registration into the appropriate classes or delay credit(s) appearing on your transcript
