LEAVE OF ABSENCE
Employees must apply for unpaid Leave of Absence (LOA) by contacting Human Resources at (904) 390-2065. Employees may apply for the following types of LOA:
· Family Medical Leave (FMLA)
· Personal Health Leave
· Personal Leave
· Professional/Educational Leave
· Military Leave
· On the Job Injury (OJI)
The following actions are required by the employee prior to the start of your unpaid LOA:
· Notify your work location of your intent to apply for unpaid LOA.
· Apply for LOA by contacting Human Resources at (904) 390-2065. Visit the Employee Central SharePoint Site or visit >Departments>Human Resources> Current Employees Under Sick Leave Pool & Extended Leave Information, select Extended Leave Paperwork.
· You must properly complete all applicable forms and provide all documentation required by Human Resources for an approved unpaid leave of absence.
· Contact the Payroll Department at (904) 390-2022 to advise the Payroll Technician assigned to your work location of your intent to go on LOA.
· Contact the Employee Benefits Department at (904) 390-2351 to make arrangements to continue insurance premiums/ coverage while you are on LOA.
Employees on leave of absence are eligible for certain benefits depending on the type of approved leave:
· Employees on LOA may elect to drop benefit coverage at time LOA begins and reinstate any dropped coverage’s upon return to work. You must notify the Employee Benefits Department within 30 days of your LOA beginning to drop benefits or 30 days of your return to work to reinstate benefits.
· If you are on a Board-approved, FMLA Leave of Absence, the employer paid health plan will continue for 84 days and life insurance for 30 days from the effective date of the leave.
· If you are on a Board-approved, non-FMLA unpaid leave of absence, your employer-paid group health plan and life insurance will continue for 30 calendar days from the effective date of the leave.
· Employees approved for On-the-Job Injury/Worker’s Comp will continue to have employer paid basic health and life insurance coverage as long as the leave is classified as OJI/WC.
· Fringe Benefits Management Company (FBMC) will mail you a payment schedule for continuation of health benefits and flexible benefits insurance premiums that you wish to continue. If you have any questions, contact FBMC at 1-855-569-3262.
· It is your responsibility to make payments to FBMC for your benefit coverage’s while on LOA and if payment is not received by the 10th of the month, any coverage’s not paid will be terminated back to the end of the prior month.
· Dependent Care FSA cannot be continued while on Board approved unpaid LOA.
