Transcript Request Form

THIS FORM IS FOR STANTON COLLEGE PREPARATORY SCHOOL STUDENTS ONLY. If you attend another high school, please contact your School Counseling Guidance Office. Use this form to request your high school transcript to be sent to a college, scholarship, or for personal use. Please only request ONE transcript per form. If you need to request more than one transcript, complete an additional form.

1.	Enter your Duval County student ID number without the leading "S." If you are a graduate, enter 99.*

2.	Enter your last name, first name*

3.	Enter a valid email address that we can use if there is a problem with your request.*

4.	What is the year of your high school graduation, e.g. 2015, 2016, etc? *

5.	Enter the name of the college or scholarship*

6.	What application portal are you using? Common App, Coalition, Institutional app, SendEdu, etc..*

7.	Which school counselor are you working with? (Ibasco, Szymczyk, Fessenden, or Watanabe)

8.	Enter the city and state of the college or scholarship. Enter the complete address if it must be mailed or if it is for a scholarship.

9.	Use this box to indicate any special circumstances, e.g. if there is a special due date, if you are picking the transcript up or wish it emailed to you, it is for personal use, SSAR, etc..
