"Teach Flu a Lesson"
Student Vaccination Program
Healthy Schools LLC, in partnership with Duval County Public Schools and the Florida Department of Health Duval County, will be again offering the flu vaccination program for our students. The Centers for Disease Control (CDC) and Advisory Committee on Immunization Practices (ACIP) do not recommend using the flu mist as a vaccine, so the only vaccine available will be administered by injection. Healthy Schools LLC trained and skilled staff of nurses will be providing the vaccinations.
The flu vaccine program will be available in all district schools on the dates and times listed in the 2017 Student Flu Vaccination Program Schedule between September 18 - October 6, 2017. A consent form must be completed and signed by a parent or guardian then returned to the school in order for the student to be eligible to receive the flu vaccine BEFORE their scheduled date and time. NO CONSENT FORM, NO VACCINE, NO EXCEPTIONS.
Along with the consent form that will be sent home with students at the start of the school year, there will be a Vaccine Information Statement from the CDC that provides more detailed information about flu vaccine. Participation in the flu vaccination program is voluntary, but we hope you will take part in this opportunity and help to keep your child and school healthy. If you would like more information about Healthy Schools LLC, have questions about the program or need assistance in completing the consent form, please call 1-800-566-0596 or go to the website on right of this page.
