    THE OFFICE WILL BE OPEN MONDAY-THURSDAY DURING THE SUMMER FOR REGISTRATIONS. THE REGISTRATION PACKET IS AVAILABLE TO PRINT BELOW. ONCE YOU HAVE ALL OF THE LISTED REQUIRED DOCUMENTS PLEASE BRING YOUR COMPLETED PACKET IN TO COMPLETE THE REGISTRATION PROCESS. 

     
    Please verify that you live in the attendance area zoned for Chimney Lakes Elementary by CLICKING HERE  to use the school locator. If you are not zoned for our school but wish for your child to attend Chimney Lakes Elementary you can apply via our magnet program. Please visit the the School Choice/Magnet tab for more information on how to apply.

    Once you have all of the required documents visit the school office to turn in your registration packet. Please note that registration packets can not be accepted without all required items.

    All registrants must provide valid documentation to include proof of birth age, Florida certification of immunization or exemption, proof of residency, and meet the following school registration requirements under Florida Law: 
    • Kindergarten - Children may enter kindergarten if they will be five years old on or before September 1st.
    • First Grade - Children may enter first grade if they will be 6 (six) years old on or before September 1st, and have successfully completed kindergarten.
    • All children who will be 6 (six) years old by February 1st must attend school.
    • All children must attend school until they reach the age of 16 (sixteen).
    The requirements above are Florida law and there are no exceptions.
     
     

    -Birth Certificate
    -Immunization Record (must be on Form 680)      School Health Entry Parent Letter
    -Physical dated within last 12 months
    -Proof of Grade Level (student’s last report card or school document stating grade level - N/A for New KG Enrollment)
    -Evidence of Custody/Guardianship (if the person registering is not the parent)
    -Photo ID of person registering student

    -Proof of Residency Document 1 – one required
    • DRIVERS LICENSE (SHOWING CURRENT ADDRESS)
    • JEA BILL
    • MORTGAGE STATEMENT
    • LEASE AGREEMENT (PRIVATE LEASE MUST BE NOTARIZED & TWO ITEMS FROM NEXT SECTIONS ARE REQUIRED)
    -Proof of Residency Document 2 – one required (dated within last 30 days)

     

    • Auto Insurance
    • Credit Card or Bank Statement
    • US Postal service confirmation of Change of Address
    • Payroll Statement
    • Non-cellular phone bill
    • Vehicle Registration

     

     

    -Proof of Residency Document 3 – one required if Document 1 was a private lease (dated within last 30 days)

     

    • Auto Insurance
    • Credit Card or Bank Statement
    • US Postal service confirmation of Change of Address
