7th Grade Immunization Requirements
Parents of incoming 7th graders. Tdap forms are available in the main office for pick-up and drop off or the forms can be downloaded and filled out.
Per Florida Statute 1003.22, all 6th graders must have a Tdap vaccine documented in Focus and be coded with an immunization status of “Permanent Immunization Documenting Seventh Grade Requirements [8]”, prior to the start of the new school year when they enter 7th grade.
Students who do not meet this state mandate or do not have all required immunizations for 7th grade, should not be allowed to start school until the required immunizations or appropriate documentation have been submitted to the school, unless they have provided the school with one of the following exemptions:
- Temporary Medical Exemption on a DH680 form (with an expiration date that is current)
- Permanent Medical Exemption on a DH680 form
- Religious Exemption on a DH681 form
