Requirements for Enrollment

Enrollment Hours: 7:00 am - 1:30 pm

Monday - Thursday

(Subject to change during the summer)

Students transferring from a Duval County Public School

Must provide the following at the time of registration:

• Enrollment Forms (completed by parent/guardian)

  • Printable Enrollment Form (English)

  • Printable Enrollment Form (Spanish) Spanish Version

• Drivers's Licenses or State ID

• (2) Proof of Address (Must be in the name of the Parent or Legal Guardian dated within past 30 days

  • 1 (one) document from the sources listed in Column A

  • 1 (one) additional document from Columns B or C (both sources of information must match)

• Withdrawal Form

• Birth Certificate (if not in FOCUS)

• Form DH680 (Shot Record) (if not in FOCUS)

Students transferring from Out-of-County / State or NON-Public School

Must provide the following at the time of registration:

• Enrollment Forms (completed by parent/guardian)

• Drivers's Licenses or State ID

• (2) Proof of Address (Must be in the name of the Parent or Legal Guardian dated within past 30 days

  • 1 (one) document from the sources listed in Column A

  • 1 (one) additional document from Columns B or C (both sources of information must match)

• Withdrawal Form

• Transcripts and most recent report card (during the school year)

• Birth Certificate

• Form DH680 (Shot Record) (if not in FOCUS)

• Physical examination for all students who have never attended a Florida public or private school

  • (Physical must be no more than 12 months old at the time of the student's enrollment)

• Records Request Form

  • Printable Records Request Form Records Request Form

*Private Lease agreement must be notarized AND two additional documents from column B and column C are required.
