Required Documentation & Uploading Forms
To play a sport, you must UPLOAD:
Sports Physical form for the current school year (For example, 25-26 school year) signed by a doctor & parents
Health Insurance,
Birth Certificate
Consent and Liability Release Form
Students Must Have a 2.0 GPA to Participate in Sports
Please note: All tryouts and practices are CLOSED to parents or spectators. Paperwork is due to the Athletic Director before participating in tryouts.
