To play a sport, you must UPLOAD:

• Sports Physical form for the current school year (2026-2027) signed by a doctor & parent

• Health Insurance

• Birth Certificate

• Consent and Liability Release Form

• Students Must Have a 2.0 GPA to Participate in Sports

  ALL DOCUMENTS MUST BE UPLOADED TO THE AKTIVATE WEBSITE

Please note:  All tryouts and practices are CLOSED to parents or spectators. Paperwork is due to the Athletic Director before participating in tryouts.
