Participating in Middle School Athletics
For all athletes interested in trying out for sports at Springfield Middle School, you must complete the following required forms and upload them to your Aktivate account prior to trying out for any sport. Be sure to note which student category represents you. Once you have completed these forms, they are good for the entire academic year.
EL2 (Physical Exam)
EL3 (Consent and Release)
Birth certificate
Proof of Insurance
NFHS Concussion Course, NFHS Heat Illness Prevention Course, NFHS Sudden Cardiac Arrest Course, NFHS Sportsmanship Course (located on Aktivate)
If your student is trying out for multiple sports, the sports may be selected in the student's profile.
In order to participate on a Middle School Athletics team students must be registered in Aktivate and upload updated documents, each year.
