Florida Health - Duval County

In addition to compliance with all required immunizations, students entering, attending, or transferring to 7th grade in Florida schools must have the following vaccines to start 7th grade:

  • Tetanus-diphtheria-pertussis (Tdap) vaccine

If your child has already received these immunizations, you must provide the school with proof on a fully completed Florida Certificate of Immunization form (DH 680).

If your child has not received these immunizations, please contact your child's medical provider or a Health Department Immunization site right away.

Only those students who have documentation of completing these immunization requirements (or have documentation of a valid medical or religious exemption) will be allowed to start 7th grade.

These two locations offer, by appointment only, immunizations, religious exemptions, shot records transcribed (from another state) and translated (from another country).

Appointments can be scheduled for Monday-Friday, 8:00am until 4:00pm. No patients are scheduled between 12:00pm-1:00pm.

Parents may drop off shot records for transcription or translation at any time. Allow at least one week for shot record pick-up.
