A Reminder to ALL 6th Grade Parents - Immunizations: In addition to compliance with all required immunizations, students entering, attending, or transferring to 7th grade in Florida schools must have the following vaccines to start 7th grade: Tetanus-diphtheria-pertussis (Tdap) vaccine If your child has already received these immunizations, you must provide the school with proof on a fully completed Florida Certificate of Immunization form (DH 680).
